Frequently asked questions about maxi and mini abdominoplasty

The abdominoplasty procedure can help men and women with skin laxity and loose abdominal muscles get a flat stomach. The laxity may be the result of child births or weight fluctuations. When this problem is limited to the lower abdomen we can treat it with a mini abdominoplasty procedure which is less extended.

What is our goal?
With abdominoplasty we aim at fixing the abdomen aesthetically but also at improving its muscular function.

What is the preoperative procedure?
The systematic exercise of the abdominal muscles contributes to a good result post surgery, but the weight loss-in case you have excess weight- also helps. Smoking, overconsumption of alcohol, use of aspirin and vitamins two weeks before the surgery should be avoided.

How is abdominoplasty performed (techniques, description of the process)?
As mentioned above, there are two techniques: the classical abdominoplasty with which we also reposition the navel and the mini abdominoplasty with which the navel is not repositioned, the incision is smaller and the operation is shorter. In both operations the incision is made on the upper border of the pubic area. When we perform the mini abdominoplasty the incision is limited to that area. On the contrary, with the maxi abdominoplasty we can get to the border of the thighs. In any case the incisions should be hidden in your underwear or the beachwear. If required, the abdominoplasty can be combined with liposuction on the same and the surrounding areas for better results.

The type of anaesthesia
The mini abdominoplasty can be achieved with epidural anaesthesia, whereas the maxi with general anaesthesia.

How long does the operation take? How long will I stay in the hospital? Is the downtime short? And finally, when will it be safe for me to go swimming without anyone noticing any signs on my body?
It all depends on the operation and the surgeon’s experience. The mini abdominoplasty takes almost 1 hour and the maxi 2 hours. This time is longer if the tummy tuck is combined with liposuction on the affected areas.

When the abdominoplasty is completed the patient goes home the same day. In case of a maxi abdominoplasty, the patient usually stays at the hospital overnight.
After the mini abdominoplasty has finished, the downtime does not last more than 4-5 days. However, after an extended abdominoplasty which has been combined with liposuction, the downtime is longer and lasts almost 2 weeks.

The oedema and the swelling subside within 15 days and if the healing is good you will not see any signs as the incisions will be hidden in your underwear or beachwear. However, we recommend you not to swim earlier than 4 weeks, as this may affect the sutures (both internally and externally). Of course, you may have a quick swim after 2 weeks, but you must be careful: Any abrupt movements can affect the sutures.

After how long can I be exposed to the sun? How long will the result last?
Any limited and careful sun exposure is not considered to be a contraindication. However you must protect the area of the incisions which should never be exposed so early. The result is permanent as long as you maintain a healthy weight and exercise.

Are there any risks and side effects? Who should not undergo such a procedure?
The risks and the complications that may occur are generally common in all surgical procedures but they are also rare. For example they may include haematoma, inflammation, bad healing, asymmetry, accumulation of fluid, skin necrosis and very rarely deep thrombosis and pulmonary embolism.
Those suffering from serious health problems, such as diabetes, heart disease, history of embolism, obesity etc should not undergo the abdominoplasty.

What can I do about love-handles?
We call love handles those areas at the side of the waistline. They respond very well to liposuction and you have a firmer middle.

Rejuvenate your eyes, neck, cheekbones, lips, hands with just 1 injection

1.The eyes
As time passes, the face loses its volume gradually and you start noticing signs of aging around the eyes. There are hollows on the upper and lower eyelids. This problem co-exists very often with black circles. We can treat all the above with hyaluronic acid injections in these areas or even with the new generation collagen.

2.The neck
What kind of procedures should we do for the neck?

The tightening can be achieved with anti-wrinkle and muscle relaxant injections alone or in combination with hyaruronic acid which fills the vertical lines. On the other hand, there is mesotherapy which offers hydration. When there is some localised fat-that is the double chin-we can treat that with phosphatidylcholine injections.

The ideal candidates
In the neck area there is the platysma muscle. When it contracts, it affects almost all the movements of the neck and the lower jaw. This gradually results in laxity of the neck area and the face contours. People often notice vertical bands as well as the double chin that can co-exist with skin laxity.

What will change?
We achieve the lift of sagging skin in the middle of the neck, a problem we notice even in young people. The procedure also improves the face contour targeting at an oval shape. The elimination of the double chin offers the ideal right angle of the neck profile.

What is the procedure?
We apply anti-wrinkle and muscle relaxant injections along the loose area of the platysma muscle in the neck and along the contour of the lower law (this method is called Nefertiti Lift, reminding us of Nefertiti, the queen of Egypt with an ideal neck).

How long does it last?
A few minutes.

What we should be careful about later?
You should not massage the area of where the anti-wrinkle and muscle relaxant injections were applied for 4 days.

Signs/scars
They are very few and go away in a little time.

Duration of the result
The result of the anti-wrinkle and muscle relaxant injections lasts at least 6 months and phosphatidylcholine is almost permanent.

Side effects and drawbacks
There may be some difficulty to swallow, but this is rare. Phosphatidylcholine injections cause mild swelling in the area for a few days and a feeling of “unwellness” that goes away very fast.

3.The cheekbones
What do you suggest someone to do in order to improve her cheekbones?

Skin laxity affects the tissues above the cheekbones and this makes them seem droopy. As defined cheekbones are one of the most youthful characteristics, their reconstruction rejuvenates the face.

How does this happen?
By injecting dense hyaluronic acid in the deeper layers of the skin or near the bones.

How long does this last?
15′- 20′.

What should we be aware of afterwards?
Nothing in particular.

Signs:
Maybe you have some bruising and mild swelling.

Duration of the result:
6- 12 months.

4.The lips
What do you suggest we should do on the lips with an injection?
Hyaluronic acid and particularly its less dense formula is an ideal material, as well as the new generation collagen which is very malleable, safe and technologically advanced (the old generation drawbacks included short duration, allergic reaction, mixtures, etc).

The ideal candidates:

5.The hands
What kind of treatment would do suggest for the aging hands?
The loss of volume concerns our hands which seem skinny as we grow older. The injectables (hyaluronic acid) are the best solution.

What is going to change?
The hands regain their volume and a better shape, which is much more attractive and youthful.

In general, there is not a specific age for these operations, as the needs are customized and every woman is different. The indications are determined by specific scientific criteria in every case and by the woman’s desires and needs. Our genetic material plays a great role, as well as our lifestyle and the things that have influenced our life.

Does crisis in a marriage or a relationship lead to the plastic surgeon’s office?

When a marriage or a relationship are in crisis, the solution that comes first in a woman’s mind is a visit to the plastic surgeon. Why? Perhaps because we seek ways to rejuvenate and refresh ourselves.

Most of the times we take this decision very late, not for the relationship but for ourselves. Of course the solution to keep the relationship alive is not in the hands of a plastic surgeon. We should disconnect these two things and see them separately. Apart from the bad mood we may be in, there are aesthetic problems we want to solve. If we achieve that, then our mood will be much better.

We overcome the sentimental difficulties and we are in a phase of rejuvenation. However, we should realize that this will not solve the problems with our partner. We must also take into account that when we decide to do an aesthetic procedure, the downtime (especially if it is a surgical operation) requires patience and energy-no matter if it is short.
This will help us overcome our sentimental stress and have a good recovery time.

Economic crisis and plastic surgery

Does the economic crisis stop people from being vain and wanting to look younger with plastic surgery? On the other hand do scientists propose us any solutions that allow us to offer ourselves such small luxuries?

Has the International Monetary Fund and financial crisis hit the market of plastic surgery?
“I think that a problem affecting a whole country so much cannot but also affect every professional and industry. On the other hand, we do not see a similar decline in plastic surgeries as we notice in other industries, and this can be explained by the fact that in such economically hard times people seek ways to boost their psychology. One effective way with long lasting results is the improvement of our appearance. When we look ourselves in the mirror and we look beautiful, our mood and psychology are boosted! Talking from experience, as I also lived the first wave of the economic crisis back in 2008 in London (I had been working there from 2002 until recently), I noticed the same phenomenon. I had patients who had been fired but put their money aside-thanks to income from other sources-in order to improve their appearance and look beautiful when going on job interviews”.

Given the economic crisis, is it possible for a woman to have low budget treatments? Whichare they?
“The low budget therapies include the injectables, mesotherapy for the face and the body, microdermabrasion, chemical peels or laser for the improvement of the skin texture. What we finally do depends on the patient’s problem. However, there are also low budget operations, for example if someone has extended skin laxity on the abdomen, the surgery solves the problem. You save money by doing only one procedure and by avoiding many sessions with one device and diets which may not offer the best solution”.

Have you adapted to what happens today and if yes, in what way? Do patients ask for discounts?
“Everyone should adapt to the new socio-economic situation and this applies to the prices too. If patients with whom I have a good trustworthy relationship ask me for a discount and they really need it, I could do this for them”.

Which surgical operation was mostly affected by economic crisis?
“I can’t decide which is this, but I don’t really think there was one specific operation which was affected. On the contrary, I see that people prefer a combination of treatments when they need it, because they get a great discount in the second and third procedure. As prices fall, patients have the opportunity-with the budget and the research necessary- to get the treatment they want”.

Dr Villy Rodopoulou on “Myths and reality about the injectables”

Dr Rodopoulou, when we are referring to injectables, what are the key substances we are talking about and what are they doing?

“We are referring to those substances which can be injected in the face and the body. These include the anti-wrinkle and muscle relaxant injections which prevents facial mascular contraction leading to smoothing out of wrinkles as well as hyaluronic acid in gel form which improves static wrinkles and volume loss. They also include hyaluronic acid and vitamins in liquid form which are used in mesotherapy in order to boost skin elasticity. Finally the substances also include phosphatidylcholine, which melts fat and improves cellulite”.

Can someone start the injectable treatments at a certain age?
“There is no specific age for this, but there are certain indications. The injectables are mainly required in our 30s. However, someone may need to have such a treatment earlier, for example a 25 year old woman who needs a refreshed face or wants to improve the cellulite she has”.

In which cases are the anti-wrinkle and muscle relaxant injections used? In the press we read that the famous actress Sharon Stone tried Botox to fix her child’s hyperhidrosis on feet.
“The usual and more regular use of the anti-wrinkle and muscle relaxant injections is the smoothing out of facial and neck dynamic wrinkles (expression wrinkles) but its action extends to the control of hyperhidrosis as well as the treatment of muscle spasm (for example blepharospasm, torticollis, migraine)”.

Is a face with no wrinkles the goal or is this a medical mistake?
“The face is the most visible and exposed body part, our mirror, and this is why it should always be in harmony with the rest of the body and our age. A refreshed face is not motionless or plumpy/swollen. It should always be in harmony with the body. Our eyes are trained to recognize what is not normal. The latter is never attractive. This is why to my mind exaggerations are inacceptable, even if the patient asks for them”.

How can you decide the ideal quantity allowing the face and the patient to make deep wrinkles disappear?
“This procedure is usually gradual. The first time that I will perform the treatment on a patient, I will give the injection which, according to my experience, will offer a satisfactory result. The patient returns after two weeks that the action of the substance has been completed and then I check the result with her. If necessary, I add some material and improve the result keeping into account that it should look natural. The reaction and the problem of every patient is unique and this is why the treatment should be customized and gradual. I prefer the most conservative dosage, because I can add but not remove in case of an unnatural result. I always write down the patient’s medical history in every detail, what I have done and I keep a personal photographic archive. In this way I know the next time what I must do in every detail”.

In how many days will I see the results?
“The action of the drug usually starts within 3-4 days and is completed in two weeks”.

Are there women in the media who are over 30 years old and have never tried an injectable treatment?
“Yes, why not? Genes and lifestyle play a very important role in our looks. A person after 40 may also need to get injections no matter if he/she is working in the media”.

Who shouldn’ t do them?
“The contraindications are very few and that is why injectables are a very famous treatment. They should mainly be avoided by patients with autoimmune diseases of the muscles or by those who are in a pharmaceutical therapy with antibiotics”.

Tell us about hyaluronic acid. Where do you use it and under what conditions?
“Hyaluronic acid is a natural substance of our body. To be more specific, it is a hydrophilic protein, component of the true skin or chorion. Hyaluronic acid used in the injectable treatments is made in the laboratories and it is biocompatible.

We use it to improve the wrinkles or the volume loss on the face, neck, décolleté, hands etc. We mainly inject it in the lower third of the face to improve nasolabial folds, starting from the base of the nose and extend to the outer corner of the mouth, the marionette lines starting from the corners of the mouth to the chin, the correction of the deep wrinkles on the glabella, the smoker lines (the vertical lines around the lips) and the under eye hollows. It also helps us add volume to the lips and improve their contour. We use it to add volume to cheekbones and define the facial contour too”.

What complications can someone have from the injectables?
“The most usual are mild oedema which lasts 1-2 days and maybe some bruising which goes away in a few days. Of course they are covered with makeup. In case the patient can touch it, we can easily smooth it out with massage”.

We often notice that when some celebrities smile their face remains frozen. Why does this happen?
“I am against any exaggerations which give a fake look, especially to those people who are public figures. These examples should be avoided. The ideal result is something natural which is not visible by non-professionals”.

When a woman gets a facelift, should she continue receiving injections?
“Yes, she may have to go on with them, especially with the anti-wrinkle and muscle relaxant injections, because dynamic wrinkles do not usually improve with plastic surgery”.

Which specialties can perform injectable treatments?
“Any experienced and well trained plastic surgeons and dermatologists”.

Up to what age can a man or a woman use injectables;
“As we said age is something so personal and it may have nothing to do with the way a face looks. There are men and women who have a very youthful skin even in their 50s, because they get injectable treatments early enough so that their wrinkles do not seem worse”.

Women seem to have loved a lot these substances. True or false?
“Maybe that’s true, as women are more sensitive with their looks. However, the last years you will see lots of men going to a doctor’s office to improve their appearance with injectables”.

To conclude, please mention some misinformation about injectables.
“There was and still is a lot of misinformation about injectables mainly about botulinum toxin. After 20 years that this pharmaceutical substance has been used and has been proven to be safe and effective, the best way to be convinced is to visit a plastic surgeon or dermatologist performing the injectables and ask to be informed”.

Some myths about them are the following:
1. Do injectables change our characteristics?

Only their misuse.

2.If you stop the injectable treatment, will your face be worse than before?
This is not true! On the contrary, the systematic treatments improve the face and offer long lasting results, even if you suddenly stop them.

3. The more we cancel the treatments the better?
No, the opposite is true. Starting early is a kind of prevention and offers better results.

4. Are these treatments costly?
No, they are very economical.

Get rid of cellulite with liposculpture

Women with localized fat who resort to liposuction can now have smooth skin without the orange peel look, thanks to the CelluTite tip.

Until recently doctors gave a negative answer when asked if they can treat severe cellulite with surgery. But now liposuction can knockout cellulite. As plastic surgeon Dr Rodopoulou explains, CelluTite is the new cannula of the BodyTite device (RF liposuction), which allows the doctor to insert it safely close to the upper layers of the skin, just where cellulite is. On the contrary, the common cannula goes deeper, otherwise the plastic surgeon would risk causing dimples.

CelluTite has an inner forked cannula emitting radiofrequencies which liquefy the subcutaneous fat. The doctor uses this after he/she has completed the liposuction procedure with the common cannula and uses it close to the areas with cellulite: inner thighs, buttocks, arms and abdomen. According to Dr Rodopoulou, with this specific cannula even a severe problem is improved up to 80-90%. However, it is difficult to guarantee the elimination of cellulite.

Does liposuction cause cellulite?

Some days after the liposuction you had in order to get rid of the localized fat, you notice the orange peel look on your hips. What caused that? Is it true that liposuction may lead to the creation of skin creases as well as of other signs giving it this uneven texture?

During the downtime period, some women realise that their hips may be thinner, but they have dimples and uneven texture which looks like cellulite. According to many doctors this is not due to liposuction.

The orange peel look appears on the upper layer of the epidermis, where the plastic surgeon does not intervene. The factors that cause cellulite are genetics, hormones, lack of exercise and bad nutrition (fast food, sugar etc). That’s why this problem also affects thin women.

What the expert says
According to plastic surgeon Dr Rodopoulou “the dimples and the uneven skin surface that a woman may notice after liposuction are not cellulite. They are unwanted side effects which don’t occur often and are not expected. In fact they happen when the procedure is not performed carefully and there is excess fat removal from an area of the body. The creases may occur when there is skin laxity and may become more visible after liposuction”.
Dr Rodopoulou adds: “In any case, the liposuction procedure which is performed with the BodyTite radiofrequencies and the CelluTite tip helps a lot in these situations, as the doctor places it more superficially in order to treat cellulite”. Finally, as Dr Rodopoulou mentions, the dimples can be treated e.g. with injecting a filler.
